The reward engine

It runs lean on dopamine

PET imaging finds fewer dopamine markers in the reward core of the ADHD brain: fewer transporters and D2/D3 receptors across the nucleus accumbens, midbrain, and caudate.

The size of that drop tracks how severe the inattention is. So the reward signal meant to say “this matters, keep going” reaches the brain quieter than it should.

Dopamine is the price of effort

Two tracts carry that signal up from the midbrain: the mesolimbic path to the reward core, and the mesocortical path to the thinking brain.

Dopamine here does more than register pleasure. It encodes the value of work: how willing the brain is to spend effort right now. Run it lean, and starting feels expensive long before the task gets hard.

The same behavioural fingerprint shows up across every model of ADHD: steep delay discounting. Future rewards lose their pull fast.

A payoff ten minutes away can feel worthless next to something now. So “it matters for the exam in three weeks” lands flat, and the brain reaches for whatever pays out now instead.

Attention is state-dependent, not broken

The prefrontal cortex runs planning, pacing, and self-control. In ADHD it runs under-powered. One result reframes what that means:

Under-stimulated, by default

The Moderate Brain Arousal model puts the ADHD brain below its best arousal level, so it goes looking for more signal to climb back up.
